Overview of Opportunity: 

Responsible for working with the Regional Medical Officer to develop a sustainable program for engaging the most frail, chronically ill population. Will help design CareMore's national strategy on engaging high risk members. Responsible for serving as the primary care physician for a subset of CareMore members deemed to be high risk. Through intensive outpatient management, responsible for key performance indicators associated with appropriate high risk management, including ER visits, inpatient admissions and re-hospitalizations. Leverages the full complement of CareMore resources, own clinical acumen and innovative approaches to best serve the high acuity needs of the highest risk members. 

Primary duties may include, but are not limited to: 

  • Leads the direction of the high risk program, which includes developing protocols, determining resourcing, reporting on performance to local and national leadership, building positive relationships with community providers and overseeing the overall success and growth of the program. 
  • Maintains a high risk clinic for face to face visits with high risk patients. 
  • Responds to patients calls and diverts patients from ER to clinic as appropriate. 
  • Maintains a population-health based approach to proactively manage high risk patients. 
  • Trains CareMore Care Center (CCC) team in managing high risk, chronically ill patients. 
  • Engages social work, home health, pharmacy, behavioral health, case management, nurse practitioners, other physicians, specialists, clinical partners, outside vendors and others on a regular basis to ensure seamless, organized patient care. 
  • Performs certain urgent care functions, such as administration of IV, nebulizations, hyperglycemia management in a clinic setting. May cover night calls or weekend calls as scheduled. 




Qualifications: 
  • Requires a MD or DO. 
  • Requires current unrestricted medical licensure in applicable state(s). 
  • Requires board certification in internal medicine. 
  • DEA license required. 
  • One year of experience working with geriatric/super-utilizer patient population; or any combination of education and experience, which would provide an equivalent background. 
  • For CareMore business units, satisfactory completion of a pre-placement Tuberculosis test is a requirement for this position.
