The Behavioral Health AdvancedPractice Clinician (APC), CareMore Primary Care, will serve as the primary behavioralhealth clinician for a panel of complex Medicaid members, as part of an integratedclinical team addressing medical, behavioral, and social health issues.  This individual will focus on assessing,diagnosing and managing patients with severe mental illness, substance abuse,and other behavioral health conditions while providing oversight and clinicalguidance to the primary care team for behavioral health care.

  • Assess, diagnose, and treat individualsand families with behavioral health conditions, ranging from serious mentalillness to substance abuse, or the potential for such conditions, using theirfull scope of therapeutic skills, including the prescription of medication andadministration of psychotherapy, as appropriate.
  • A truepatient-centered approach, from a bedside manner that puts patients,caregivers, and team members at ease
  • Acollaborative team member and effective communicator
  • Willingnessto operate in an ambiguous environment, without continuous supervision
  • Experience operating in an integratedbehavioral health delivery model
 
Essential Duties & Responsibilities
  • Assess, evaluate, diagnose, and manage the behavioral health conditionsof patients
  • Performs mental statusexaminations, biopsychosocial assessments, psychotherapy, and, whenappropriate, prescribes and manages medications within prescribed guidelines and instructions of physicianleadership
  • Order, interpret, and evaluate diagnostic tests to identify and assesspatient’s clinical problems and health care needs
  • Record physical findings, and formulate a plan and prognosis, based onthe patient’s condition
  • Discuss case with the medical and behavioral health physician leadershipand other care team members(social work, therapists, care managers) to prepareand enact a comprehensive patient care plan
  • Submit the goals of individual patients for periodic review andevaluation by the physician leadership
  • Refer patients to physician leadership for consultation or specializedhealth resources for treatment
  • Maintain completemedical records, including history, physical exams, and telephone messagesummaries timely and accurately
  • Attend behavioral healthand/or medical rounds at the care center, perform e-consults as needed,collaborate with care center teams on both medical and behavioral healthaspects of care
 

Work Location & TravelExpectations

Basedin local market, with local travel. Provides nighttime, weekday or some weekend and holiday call coverage aspart of work schedule.


Job Requirements

  • 1 year of experience as a Nurse Practitioner
  • 3-5 years practice as abehavioral health clinician strongly preferred
  • Current RN and Nurse Practitioner license (FNP/ANP/GNP) in good standingwith the state in which you are practicing
  • Masters of Science in Nursing
  • Board Certified either with AANP or ANCC
  • Psychiatry Training and Psychiatric Mental Health Nurse Practitionerboard certified required.
  • Prescribing authority/DEA licensure
  • NPI billing number
  • Optional: Bilingual(Spanish or French)
