Responsible for the oversight and management of the Clinical Programs.

Primary duties may include, but are not limited to: 
  • Oversees and manages Chronic Disease Management Programs
  • Coordinates care and treatment of patients with chronic conditions.
  • Researches and develops new and innovative approaches for care and treatment of chronic conditions.
  • Provides resources and direction to Nurse Practitioners. 
  • Evaluates clinic staffing and scheduling to prevent delays in patients accessing services and proper utilization of resources. 
  • Utilizes clinical knowledge and expertise to evaluate care provided in each specialty area. 
  • Hires, trains, coaches, counsels, and evaluates performance of direct reports. 

Job Requirements

  • Requires a MS in Nursing; 5 years of experience with clinical emphasis for nurse practitioner; or any combination of education and experience, which would provide an equivalent background.
  • Current unrestricted RN license and NP license in applicable state required. National Board Certification  
  • Prescribing authority/DEA licensure in good standing as required in the state in which you are applying preferred.
  • NPI billing number
  • Medicare Part D Prescriber
  • 2-5 years of experience working in acute care settings such as: ICU, CCU, telemetry, med-surgical is highly preferred.
  • Experience in a supervisory, management role is a plus.
  • Must be flexible with local traveling
