Location:  This position is a Work-At-Home Opportunity. Must reside in the Memphis/Western area of Tennessee for close proximity to assigned territory.

 
 

The Health Promotion Consultant (Medicaid) is responsible for member education, outreach, collaboration and coordination of screening events with contracted providers at provider clinics and community settings to ensure members have access to care, close gaps in care and maintain compliance with Tennessee state mandated cultural and linguistic, health education requirements, or other programs required by contracts. This role works directly with contracted providers/physicians to increase Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) screening rates and other HEDIS measures by utilizing the clinic day process. Primary duties may include, but are not limited to:

  • Support Contractual Requirements related to TennCare Kids (Tennessee’s Early and Periodic Screening, Diagnostic, and Treatment Services (EPSDT) Program.

  • Review HEDIS metrics, clinic day screening outcomes and provide outreach support. 

  • Identifies and develops key stakeholder criteria, creates relationships with desirable community partners, determines availability of local programs and opportunities for increasing participation to close gaps in care.

  • Provides/promotes provider training to assure compliance with government mandated health education programs and the health education referral system.

  • Establishes positive relationships with community based organizations and other key partners to gain understanding of the cultural and linguistic needs of members.

  • Recommends strategies and interventions based on analysis of State/Regional/Local health statistics, disease prevalence, census data, and noncompliant population data to improve member compliance and participation.

  • Maps community based assets and collaborates with key stakeholders to leverage available resources for development of new and accessibility to existing health education/promotion programs.

Job Requirements

Requires:
  • BA; 2 years of experience; or any combination of education and experience, which would provide an equivalent background.
  • Certified Health Education Specialist (CHES) a plus. 
  • Knowledge of HEDIS metrics, and SalesForce preferred.
  • Day travel 75% of the time.
  • Some holidays and weekends required.
