The Behavioral Health Case Manager position supports our Medicaid members with behavioral health issues. Experience with children with serious emotional disturbance, persons with substance use disorders, and/or persons with serious mental illness is highly preferred. 

As the Behavioral Health Case Manager you will be responsible for performing telephonic case management within the scope of licensure for members with behavioral health and substance abuse or substance abuse disorder needs. 
  • You will be using appropriate screening criteria, knowledge and clinical judgment to assess member needs. 
  • You will conduct assessments to identify individual needs and develops care plan to address objectives and goals as identified during assessment. 
  • The BH Case Manager will monitor and evaluate effectiveness of care plan and modifies plan as needed. 
  • You support member access to appropriate quality and cost effective care.
  • As the BH Case Manager you'll coordinate with internal and external resources to meet identified needs of the members and collaborates with providers.  

The BH Case Manager may be hired as a BH Case Mgr. I or II depending upon the skills, background and exp. of the chosen candidates.
Required qualifications:
  • Requires MS or MA in social work, counseling, nursing or related behavioral health field. 
  • Your experience must include a minimum of 3 years' clinical experience in social work, counseling with a broad range of experience with complex psychiatric and substance abuse or substance abuse disorder treatment; or any combination of education and experience, which would provide an equivalent background.
  • Requires Community Mental Health experience; CSB (Community Service Board) experience, or similar. 
  • Current, active, and unrestricted LCSW, LPC, or RN license in the state of Virginia.
  • You must live in VA and be able to travel locally; this position requires a reliable vehicle, current auto insurance and clean driving record (mileage, tolls reimbursed)
  • You must be competent in MS Office suite to include Word, Outlook and Excel, as well as comfortable with computers in general (i.e. Apple products) in that you will be entering information into an iPad and syncing that data with a Laptop. Lack of computer skills may affect your ability to be successful in this position.

Preferred qualifications:
  • Experience in case management and telephonic coaching with members with a broad range of complex psychiatric/substance abuse and/or medical disorders preferred. 
  • Experience working with children, persons with substance abuse disorder, serious mental illness, and the ID/DD population is highly preferred. 
  • Demonstrated ability to communicate with members who have complex medical needs and who may have communication barriers
  • Current or prior field/home health experience preferred
  • Knowledge of Medicaid and/or Managed Care preferred
