Responsible for assisting physicians in treating patients in home and clinic setting. Primary duties may include, but are not limited to: 
  • Explains procedures and administers prescribed medications and treatments to patients. Including wound care and alternate methods to comfort patients, such as massage.
  • Prepares equipment and aids physician during patient treatment and examination. 
  • Observes patient, records significant conditions and reactions, and notifies supervisor or Physician of patient's condition and reaction to drugs, treatments, and significant incidents. 
  • Takes temperature, pulse, blood pressure, and other vital signs to assess the patient's condition. 
  • Documents nursing history and physical assessments, and initiates a patient education plan according to the individualized needs of the patient.
  • Rotates through various clinical services and perform duties such as preparing rooms, sterilizing instruments, equipment, and supplies for surgery. 
  • Will work in clinic setting two half days per week (wound care patients) and palliative home care patients other days. 
  • Other add hoc responsibilities as needed including HEDIS follow up via phone call.  

Job Requirements


Requires a BS in Nursing; 5 years of experience; or any combination of education and experience, which would provide and equivalent background. Current unrestricted RN license in applicable state required. 


Must have strong wound care skills

Must be well versed in palliative technique's, such as alternative methods to calm anxiety.  


Must have reliable transportation to do home visits (mileage reimbursed).
