The Nurse Practitioner (NP) is responsible for evaluating and treating physical and psychosocial health needs of the adult and geriatric patient in the skilled/subacute setting of Skilled Nursing Facilities or within the patients home.  The Transitional NP is responsible for seeing all patients at least once per week, ideally more frequently, and within 72 hours of admit. Responsible for providing health services to patients in collaboration with the physician. Primary duties may include, but are not limited to:
  • Examines patients and performs comprehensive physical examinations. Compiles patient medical data, including health history and results of physical examinations.
  • Administers or orders diagnostic tests, such as x-ray, electrocardiogram, and laboratory tests, and interprets test results.
  • Performs therapeutic procedures, such as injections, immunizations, suturing and wound care, and managing infection.
  • Develops and implements patient management plans, records progress notes, and assists with continuity of care.
  • Instructs and counsels patients regarding compliance with prescribed therapeutic regimens, normal growth and development, family planning, emotional problems of daily living, and health maintenance.

Job Requirements

Education and/or Experience: Master’s degree in Nursing with emphasis in Family, Adult, or Gerontological practice. Certificates, Licenses, Registrations: Current registered nursing license and Nurse Practitioner license in good standing with the state in which you are applying.  National Board Certification and DEA license.

Home Health and Wound care experience highly preferred.
